The Basics of the Person-Centered Description

The person-centered description (PCD) is an exciting enhancement to IPMG’s Person-centered Planning Process. Each waiver recipient in Indiana has a person-centered plan (PCP), which is developed each year and utilized by your IPMG team and support providers. This unique, person-centered process, developed by IPMG, enables all members of your team to more closely align their support with your unique needs and desires for the upcoming year.

The addition of a person-centered description (PCD) to your person-centered plan focuses the support you receive through your waiver on your needs, goals, and dreams more than ever before. Additionally, the inclusion of a PCD in the PCP offers more involvement and input by the person receiving waiver services, and utilizes person-centered thinking as a base for all actions.

The result: a person-centered plan that more accurately reflects your desired outcomes for your life, with clear objectives for your support team.

In order to further explain this exciting new enhancement, IPMG has developed “The Basics of the Person-Centered Description.” This document provides you with the basics of the process, definitions you’ll want to be familiar with, answers to questions you may have about IPMG’s advances in person-centered planning, and how these enhancements benefit all people receiving a DD, Autism, or Support Services Waiver in Indiana.
