IPMG provides case management services to all individuals receiving a Developmental Disabilities, Autism, or Support Services Waiver in Indiana. IPMG case management professionals focus and direct efforts on advocating for each individual’s needs, goals, and dreams. This focus is built into a person-centered case management model, which means all of IPMG’s activities are directed by each Consumer’s expressed dreams and requirements.  

At IPMG, providing choice to each waiver recipient is critical to the overall goal of providing person-centered case management services that make the needs of each Consumer paramount. From the start of services from IPMG, choice is continuously available to all waiver recipients.

Below is a description of the roles of some of the IPMG case management professionals who work on behalf of individuals receiving a Developmental Disabilities, Autism, or Support Services Waiver in Indiana.

Case Manager
A waiver recipient’s selected IPMG Case Manager works collaboratively year-round with the individual, his or her guardian, family, service providers, and other advocates to ensure all efforts and support assist the individual with moving toward their hopes, dreams, and goals.

As the waiver recipient’s primary source of contact, the IPMG Case Manager provides information and leadership via a variety of communication means, such as phone, email, letters and written correspondence, and scheduled personal visits with the individual. He or she collaborates with others on the waiver recipient’s team so that those who work with the individual can be certain all efforts are focused on the waiver recipient’s goals and individual support plan (ISP).

Meetings with the individual's case manager are an opportunity to discuss any needs or changes that may require an adjustment to the waiver recipient’s current plans. Additionally, the case manager and waiver recipient and/or guardian may be in contact at other times, as required, to ensure quality services are provided.

Outside of the face-to-face meetings, the waiver recipient’s case manager updates other team members on the progress of the individual's ISP, collaborates with service providers to ensure continuity of services, and assures filing deadlines are met and important records are maintained.

Annual Assessment Specialist
An IPMG Annual Assessment Specialist meets with the waiver recipient each year to review annual progress and any desired or needed changes to his/her existing person-centered description (PCD). The goal of this annual assessment is to update and revise the waiver recipient’s PCD and ISP for the coming year, and to assist with preparation for his/her annual team meeting. The annual assessment assists the team with ensuring the things that are important for the waiver recipient, such as health and safety issues, as well as the things that are important to the individual, such as dreams and goals, are reflected in the individual's PCD.

Supervisor & District Director
IPMG Supervisors are responsible for overseeing and managing a group of case management professionals within a certain district. Under the guidance of the district directors, supervisors ensure these IPMG employees provide quality effort that creates opportunities and meets the case management needs of all people receiving waiver services.

IPMG provides case management that supports all Consumers to choose and modify their waiver services as their lives grow and change. This expectation advances progress toward each waiver recipient’s real and unique needs, and helps each person build a richer, more meaningful life.
