IPMG - Intake & Assessment Facilitator

This position is responsible for initiating and following a pre-determined activity flow that includes level of care determination, initial and annual PCP facilitation, initial and annual ISP development, and writing and submitting the initial cost comparison budget.

Minimum Requirements

  • 2 years' experience as a full-time certified Indiana Case Manager
  • Meets current minimum Case Manager qualifications
  • Provide documentation as a certified PCP facilitator
  • Furnish examples of PCP's and ISP's you have written in the past 6 months
