Each year, every Hoosier receiving an Autism, Developmental Disabilities, or Support Services
Waiver participates in the person-centered planning process as part of the case management
services you receive from IPMG. Your IPMG team creates a plan of support that makes the
things important for you (such as your health and safety needs), as well as what is important
to you (such as your goals, dreams, and ambitions) the center of all activities. By placing your
needs, dreams, and life goals first, IPMG provides ‘person-centered’ case management to
each and every waiver recipient.
Attend “Making the Most of Your Waiver” to find out how to put the pieces of the person-centered
planning process together to best meet your needs! The person-centered planning
process is utilized by your entire IPMG team to continuously evaluate and meet the needs that
are important to and important for you. Come learn how your annual meeting, person-centered
description, and annual assessment fit together to create a person-centered plan of support
that meets your needs and helps you achieve your dreams and goals.
Sign up to attend a workshop near you! Please note that
waiver recipients, their parents, guardians, and loved ones are welcome to attend. Workshops
are free. Space for each workshop is limited, and registrations will be accepted on a first-come,
first-served basis.
